Yes. It is a factory-unlocked US model, so it works with AT&T, T-Mobile, Verizon and other carriers. Activate it with your carrier as usual.

Does it take a physical SIM card?

US iPhone models use eSIM. Your carrier can transfer your number in a few minutes during setup, or you can convert a physical SIM with the carrier app.

Is a power adapter included?

No. The box includes a USB-C charge cable. Any USB-C power adapter works; 20W or higher is recommended for fast charging.
